Next, type in name of seminar subject or title (up to 3 lines). Provide street address (up to 2 lines); city; state or province; zip code or postal code; and country. Provide time in 24-hour or 12-hour format (please use AM or PM if using the latter); date (XX/XX/20XX); and admission, if applicable. On back, provide up to 3 contacts (social media handles, phone numbers, etc.) for guests to RSVP.
